Optum Bank debit Mastercard®

  • Pay on-the-spot at a doctor’s office, pharmacy or other health care facility.
  • Pay a bill you receive from a doctor or other provider.
  • Withdraw funds from an ATM to reimburse yourself for expenses you paid out-of-pocket.* (ATM access is not available on all HSA products.)

Dependent debit card

You can order a debit card in the name of your spouse or dependent . This provides more options to you and your family members to access health account. To order a card for your spouse or dependent, follow these steps:

  1. Sign in to your Optum Bank account
  2. Select "Manage Debit Cards" from your dashboard.
  3. Select "Add a Debit Card", then "Order Debit Card" for anyone listed.
  4. To order a card for anyone not listed, select "Click Here". Enter the required information and select "Add Family Member." Navigate back to the "Add a Debit Card" page to order a card for the newly added family member.

Lost or replacement card

If you lost your card and would like to order a replacement, follow these steps:

  1. Sign in to your Optum Bank account.
  2. Select "Accounts" from your dashboard.
  3. Select "Debit Card" on the left hand side of the page under "Account/Plan Management".  This will show you any debit cards that are currently activated for your HSA account.
  4. Click the "Report Lost/Stolen"button to report a lost or stolen card.
